RESCUE: CONTEXT is everything…

Tactical Combat Casualty Care (TCCC) revolutionized prehospital medicine practices in 1996 by emphasizing the operational context, constraints, and threats. As a result of inappropriate civilian trauma practices in unconventional tactical settings, TCCC guidelines were developed. The military SOF, tactical law enforcement units, and civilian rescue personnel such as mountain rescue Rapid Extraction Module Support (REMS) and wildland firefighters operate in non-linear, unpredictable, and high-threat environments.

A need for CHANGE…

The challenge lies in striking a balance between compliance with a standard or context-specific best practice and the inherent operational demands of a highly variable, time-pressed, and threat-saturated operational landscape. Long-standing (inherited) cognitive processes for vertical mobility are often irrelevant or illogical in a VUCA-T2 environment (volatile, uncertain, complex, ambiguous, containing threats and compressed time). The lack of relevance is often a result of misinterpretations of vertical response gold standards compared to operational TTPs that contain mission-specific waypoints. The “friction” is often caused by the end user misinterpreting the guide or the job performance requirements (JPR).

Today’s prehospital healthcare professionals are driven by innovation, motivation, and dedication. The North American Rescue Organization embodies these qualities. Special and distinctive requirements apply to each emergency medical system, such as varying levels of terrorist threats, budgetary restrictions, and equipment limitations. To mitigate these challenges, many systems have developed specialized teams, such as tactical, hazmat, or disaster response teams. Nevertheless, conventional prehospital providers bear most of the responsibility in these situations. We can integrate (where appropriate) treatment modalities and equipment that will enable us to be better prepared for our day-to-day operations if we take lessons learned from these specialized teams and combine them with current innovations in military medicine

The possibility of life-threatening bleeding occurs every day in our communities. It doesn’t matter what kind of accident it is – a car crash, farm equipment incident, or public transportation accident, a natural disaster like a tornado, or an intentional act of violence like a shooting or bombing – uncontrolled bleeding can result in death within minutes, even before the ambulance or other emergency services arrive. During the first few minutes after a traumatic injury, a life can be saved. As a leader in providing medical equipment to bystanders and first responders in life-threatening situations, North American Rescue is leading the way. Individuals and groups may administer immediate bleeding control to victims of accidents in public buildings and populated areas using these modular life-saving aid components.
